Types of Coagulation Disorders

Coagulation disorders can broadly be divided into bleeding disorders and clotting disorders.

Bleeding Disorders

Bleeding disorders occur when the blood does not clot adequately.

They may result from:

  • Deficiency of clotting factors

  • Abnormal platelet function

  • Low platelet levels

  • Problems with blood vessels

  • Certain medications

  • Liver disease

  • Vitamin deficiencies

  • Autoimmune conditions

Common inherited bleeding disorders include hemophilia and von Willebrand disease.

Clotting Disorders

Clotting disorders, sometimes called thrombophilias, increase the tendency of blood to form clots.

A clot may develop in a vein or artery and can sometimes travel to another part of the body.

Examples include:

  • Deep vein thrombosis (DVT)

  • Pulmonary embolism (PE)

  • Certain inherited thrombophilias

  • Antiphospholipid syndrome

Some clotting disorders are inherited, while others develop because of factors such as cancer, surgery, prolonged immobility, pregnancy, certain medications, or other medical conditions.

Common Coagulation Disorders

Hemophilia

Hemophilia is an inherited bleeding disorder caused by low levels or absence of specific clotting factors.

The most common forms are:

  • Hemophilia A, associated with factor VIII deficiency

  • Hemophilia B, associated with factor IX deficiency

People with significant hemophilia may experience prolonged bleeding, spontaneous bleeding, or bleeding into joints and muscles.

Modern treatment can include replacement of the missing clotting factor and other specialised therapies.

Von Willebrand Disease

Von Willebrand disease is one of the most common inherited bleeding disorders.

It occurs because of reduced levels or abnormal function of von Willebrand factor, a protein that plays an important role in platelet adhesion and clot formation.

Symptoms may include:

  • Frequent nosebleeds

  • Easy bruising

  • Heavy menstrual bleeding

  • Prolonged bleeding after dental procedures or surgery

The severity can vary considerably between individuals.

Deep Vein Thrombosis

Deep vein thrombosis occurs when a blood clot develops in a deep vein, commonly in the leg.

Possible symptoms include:

  • Swelling in one limb

  • Pain or tenderness

  • Warmth

  • Redness or skin discoloration

Some DVTs may have few or no symptoms.

Pulmonary Embolism

A pulmonary embolism occurs when a blood clot, often originating from a deep vein, travels to the lungs.

Possible symptoms include:

  • Sudden shortness of breath

  • Chest pain

  • Rapid heartbeat

  • Cough

  • Coughing up blood

  • Dizziness or fainting

A suspected pulmonary embolism requires urgent medical evaluation.

Antiphospholipid Syndrome

Antiphospholipid syndrome is an autoimmune condition associated with an increased risk of abnormal blood clot formation.

It can affect veins or arteries and may also be associated with pregnancy-related complications.

Diagnosis requires appropriate clinical assessment and specific laboratory testing.

What Causes Coagulation Disorders?

The causes vary depending on the specific condition.

Inherited Causes

Some coagulation disorders result from genetic changes passed through families.

Examples include:

  • Hemophilia A

  • Hemophilia B

  • Von Willebrand disease

  • Certain inherited thrombophilias

A family history may provide an important clue, but a person can sometimes have a bleeding or clotting disorder even without a known family history.

Acquired Causes

Coagulation problems can also develop during a person's lifetime.

  • Autoimmune diseases

  • Cancer

  • Kidney disease

Symptoms of Coagulation Disorders

Symptoms depend on whether the condition causes excessive bleeding or excessive clotting.

Symptoms of a Bleeding Disorder

Possible signs include:

  • Frequent nosebleeds

  • Easy or unexplained bruising

  • Heavy menstrual bleeding

  • Prolonged bleeding from minor cuts

  • Excessive bleeding after dental procedures

  • Excessive bleeding after surgery

  • Blood in urine or stool

  • Bleeding into joints or muscles

Symptoms of an Abnormal Blood Clot

Possible signs include:

  • Sudden swelling of a limb

  • Pain or tenderness in a limb

  • Unexplained redness or warmth

  • Sudden shortness of breath

  • Chest pain

  • Rapid heartbeat

  • Neurological symptoms if an artery supplying the brain is affected

Sudden chest pain, severe breathlessness, coughing up blood, fainting, or symptoms suggestive of a stroke require immediate medical attention.

How Are Coagulation Disorders Diagnosed?

Diagnosis begins with a detailed medical history and physical examination.

The doctor may ask about:

  • Previous episodes of unusual bleeding or clotting

  • Bleeding during surgeries or dental procedures

  • Menstrual bleeding

  • Medications

  • Family history

  • Pregnancy history

  • Previous blood clots

  • Other medical conditions

Laboratory testing may include:

Complete Blood Count

A CBC provides information about the number of platelets and other blood cells.

PT/INR

Prothrombin time (PT) and INR assess part of the blood-clotting pathway and are commonly used in evaluating bleeding disorders and monitoring certain anticoagulant treatments.

aPTT

Activated partial thromboplastin time (aPTT) evaluates another part of the clotting system and may help identify deficiencies of certain clotting factors.

Fibrinogen Testing

Fibrinogen is a protein required for clot formation. Testing may be useful in selected bleeding or clotting conditions.

Clotting Factor Assays

Specific tests can measure the activity or levels of individual clotting factors, such as factor VIII or factor IX.

Von Willebrand Factor Testing

When von Willebrand disease is suspected, specialised tests may assess von Willebrand factor quantity and function.

Thrombophilia Testing

In selected patients with abnormal or recurrent clotting, testing for inherited or acquired thrombophilia may be considered.

Not everyone who develops a blood clot needs extensive thrombophilia testing. The decision depends on the patient's history and clinical circumstances.

Treatment of Coagulation Disorders

Treatment depends on the underlying condition and whether the primary problem is bleeding or abnormal clotting.

Treatment for Bleeding Disorders

Depending on the diagnosis, treatment may include:

  • Clotting factor replacement

  • Desmopressin in selected conditions

  • Specific medicines that help control bleeding

  • Replacement of vitamin deficiencies

  • Platelet transfusions in selected situations

  • Treatment of the underlying disease

Treatment for Blood Clots

Anticoagulant medicines are commonly used to treat and prevent abnormal blood clots.

Depending on the situation, treatment may involve oral or injectable anticoagulants.

The choice and duration of treatment depend on factors such as the location of the clot, the cause of the clot, the risk of recurrence, kidney function, bleeding risk, and other individual factors.

Coagulation Disorders and Pregnancy

Pregnancy naturally changes the body's clotting system and can increase the risk of certain clotting complications.

Some women with known bleeding or clotting disorders may require specialised monitoring during pregnancy, delivery, and the postpartum period.

Pregnancy-associated bleeding or clotting problems should be managed by an appropriate multidisciplinary medical team when necessary.

Coagulation Disorders and Surgery

Patients with known or suspected coagulation disorders may require additional evaluation before surgery or invasive procedures.

The medical team may assess:

  • Current medications

  • Platelet count

  • Clotting tests

  • Specific clotting factor levels

  • Previous bleeding history

  • Risk of thrombosis

Can Coagulation Disorders Be Prevented?

Not all coagulation disorders can be prevented, particularly inherited conditions.

However, complications from some acquired clotting disorders can be reduced by addressing risk factors such as prolonged immobility, smoking, obesity, certain medications, and other underlying medical conditions where appropriate.

People taking anticoagulants should follow their prescribed treatment carefully and should not stop or change medication without medical advice.

  • Excessive bleeding after surgery or dental procedures

  • Frequent nosebleeds or unusual bruising

  • Very heavy menstrual bleeding

  • Abnormal PT, INR, or aPTT results

  • A suspected inherited bleeding or clotting disorder
