Hematological malignancies are cancers that develop in the blood-forming tissues, blood cells, bone marrow, or lymphatic system. They occur when abnormal blood cells begin to grow uncontrollably and interfere with the normal production and function of healthy blood cells.
Unlike many solid cancers that form a mass or tumour in a specific organ, hematological malignancies often involve cells that circulate throughout the body. This makes their diagnosis and treatment different from that of many other cancers.
The three major groups of hematological malignancies are leukemias, lymphomas, and plasma cell disorders such as multiple myeloma.
What Are Hematological Malignancies?
Blood cells are continuously produced in the bone marrow from specialised cells called hematopoietic stem cells. These develop into red blood cells, white blood cells, and platelets.
Normally, blood cell production is carefully regulated. In hematological malignancies, genetic and molecular changes can cause abnormal cells to multiply excessively, survive longer than they should, or lose their normal function.
These abnormal cells may accumulate in the bone marrow, lymph nodes, blood, spleen, or other tissues.
Major Types of Hematological Malignancies
1. Leukemia
Leukemia is a cancer involving blood-forming cells, usually in the bone marrow. Abnormal blood cells can accumulate in the bone marrow and blood, affecting the production of normal blood cells.
Leukemias are classified based on how quickly they progress and the type of blood cell involved.
They include:
Acute lymphoblastic leukemia (ALL)
Acute myeloid leukemia (AML)
Chronic lymphocytic leukemia (CLL)
Chronic myeloid leukemia (CML)
Acute leukemias generally progress rapidly and require prompt treatment, while chronic leukemias may develop more gradually.
Symptoms can include fatigue, recurrent infections, fever, unexplained bruising or bleeding, weight loss, or bone and joint discomfort.
2. Lymphoma
Lymphomas are cancers of the lymphatic system, most commonly arising from abnormal lymphocytes.
The two broad categories are:
Hodgkin lymphoma
Non-Hodgkin lymphoma
Non-Hodgkin lymphoma includes numerous different subtypes, each with its own biological characteristics, expected behaviour, and treatment approach.
A common sign of lymphoma is a painless swelling of lymph nodes, particularly in the neck, armpit, or groin.
Other symptoms may include persistent fever, night sweats, unexplained weight loss, fatigue, itching, or recurrent infections.
3. Multiple Myeloma and Plasma Cell Disorders
Multiple myeloma develops from abnormal plasma cells, which are specialised white blood cells responsible for producing antibodies.
Abnormal plasma cells accumulate in the bone marrow and produce abnormal proteins known as monoclonal proteins.
Multiple myeloma can affect several parts of the body, particularly the bones, kidneys, and blood.
Possible symptoms include:
Persistent bone pain
Fatigue or weakness
Recurrent infections
Kidney problems
Increased calcium levels
Anemia
Some plasma cell disorders may not require immediate treatment and can instead be monitored carefully.
What Causes Hematological Malignancies?
There is no single cause for most blood cancers.
They generally develop because of genetic changes that occur in blood-forming cells. Some genetic changes are acquired during a person's lifetime, while certain inherited conditions can increase the risk of developing particular blood disorders.
Risk factors can vary depending on the specific malignancy and may include age, previous chemotherapy or radiation exposure, certain infections, immune system disorders, environmental exposures, or genetic predisposition.
However, in many patients, no specific cause can be identified.
Symptoms of Hematological Malignancies
Symptoms vary considerably depending on the type of blood cancer and the cells or organs involved.
Possible warning signs include:
Persistent fatigue or weakness
Unexplained weight loss
Recurrent infections
Persistent fever
Night sweats
Easy bruising or unusual bleeding
Pale skin
Enlarged lymph nodes
Persistent bone pain
Abdominal fullness due to an enlarged spleen or liver
Unexplained changes in blood counts
These symptoms are not specific to cancer and can occur with many other conditions. Persistent or unexplained symptoms should nevertheless be evaluated by a healthcare professional.
How Are Hematological Malignancies Diagnosed?
Accurate diagnosis is one of the most important steps in treating a blood cancer.
Blood Tests
A complete blood count and peripheral blood smear can provide important information about the number and appearance of blood cells.
Additional blood tests may assess kidney function, liver function, electrolytes, proteins, and other disease-specific markers.
Bone Marrow Examination
A bone marrow aspiration and biopsy may be required when leukemia, myeloma, or another bone marrow disorder is suspected.
It allows doctors to examine the cells where blood production occurs and can help determine the specific type of malignancy.
Flow Cytometry
Flow cytometry analyses proteins present on the surface or inside cells. It can help identify abnormal populations of blood cells and is particularly useful in diagnosing many leukemias and lymphomas.
Cytogenetic and Molecular Testing
Modern hematology relies increasingly on genetic and molecular investigations.
These tests can identify specific chromosomal or genetic abnormalities that may:
Confirm a diagnosis
Classify the disease
Provide prognostic information
Help select targeted treatments
Monitor response to treatment
Lymph Node or Tissue Biopsy
For many lymphomas, a biopsy of an affected lymph node or other tissue is essential to establish the diagnosis and identify the specific subtype.
Imaging
CT, PET-CT, MRI, ultrasound, or other imaging studies may be used depending on the suspected disease and to assess the extent of involvement.
How Are Hematological Malignancies Treated?
Treatment depends on the exact diagnosis, disease subtype, stage or risk category, genetic characteristics, age, overall health, and previous treatments.
Modern treatment options may include:
Chemotherapy
Chemotherapy uses medicines that destroy or control rapidly dividing cancer cells. It remains an important treatment for several hematological malignancies.
Targeted Therapy
Targeted medicines are designed to act on specific molecules or biological pathways involved in cancer cell growth.
The development of targeted therapies has significantly changed the treatment of several blood cancers.
Immunotherapy
Immunotherapy helps the immune system recognise and attack cancer cells.
Examples include monoclonal antibodies, immune-based treatments, and other specialised therapies.
CAR T-Cell Therapy
CAR T-cell therapy is an advanced form of cellular immunotherapy in which a patient's own T cells are collected and genetically modified to recognise and attack cancer cells.
It is currently used for selected hematological malignancies and may be considered in specific clinical situations, particularly when conventional treatments have not been successful.
Stem Cell Transplantation
Stem cell transplantation, also referred to as hematopoietic stem cell transplantation, can be an important treatment for selected patients with certain leukemias, lymphomas, myeloma, and other blood disorders.
The decision to proceed with transplantation depends on disease characteristics, response to treatment, patient fitness, and other individual factors.
Why Subtype and Molecular Testing Matter
“Hematological malignancy” is not a single disease.
For example, two patients may both be diagnosed with leukemia but have completely different disease biology, genetic abnormalities, treatment options, and prognosis.
This is why modern hematology places considerable emphasis on precise diagnosis and molecular classification.
Understanding the biology of the cancer allows treatment to be increasingly tailored to the individual patient.
Can Hematological Malignancies Be Treated Successfully?
Many hematological malignancies are treatable, and some can be cured.
Outcomes vary significantly depending on the specific disease, its biological characteristics, stage or risk group, response to treatment, and the patient's overall health.
Advances in targeted therapy, immunotherapy, cellular therapy, and transplantation have substantially expanded treatment options for many blood cancers.
When Should You Consult a Hematologist?
You may be referred to a hematologist if you have:
Persistent unexplained abnormalities in blood counts
Unexplained anemia
Recurrent or unusual infections
Unexplained bruising or bleeding
Persistently enlarged lymph nodes
Unexplained weight loss or night sweats
Persistent fever without an identified cause
Abnormal cells detected on a blood smear
Suspected leukemia, lymphoma, or myeloma
An abnormal blood test does not automatically mean cancer. However, unexplained or persistent abnormalities should be evaluated rather than ignored.

Hematological malignancies can behave very differently from one another. Some progress rapidly and require urgent treatment, while others can be monitored for extended periods.
